Each child must be registered individually!

In case of emergency, I hereby grant my permission to have the approved workers from Wayside Baptist Church authorize medical attention by a physician or hospitalization of my child as necessary. I do not hold Wayside Baptist Church, its staff or approved workers responsible for any injury that should occur. (The Wayside staff or approved workers will notify you immediately of any such occurrence.) I also give permission for photos of my child to be taken during this event and used for promotional purposes.
